Apply for Extensions

The way you edit this page will depend on your system for collecting inquiring extensions guests.

If you prefer they use your questionnaire entirely to schedule their appointment, you can switch all of the "Extensions Application" buttons and links to link up to your [Jotform/Google Form/etc.] webpage using "Click Actions" tab on the right.

If you'd like to embed your application directly onto this website page, you can add an embed code box (from the bottom center of the Showit builder) and paste in the HTML/embed code from your form system (Jotform/Google Forms/etc.)
